Good evening. 

I have started work on an old Urgos striking clock and I noticed the hammer head is completely smooth where I would expect to see a lump of leather. There does appear to be some leather in there but has either completely worn away or someone has smoothed it off. Should it be like this or should I drill out the old leather and put a new piece in?

Have added photos if that helps.

First things how does it sound? If there is enough that protrudes out from the brass and the leather is hard put oil on it to soften it up, it will take time for it to penetrate.  If it sounds dreadful then replace it.
